Collection, Preparation and Transportation

• Place surgical or dermatology biopsy in a covered container with formalin.
• Do not use formalin on specimen(s) intended for culture.
• If cultures are desired, use a separate container and label it “for culture.”
• All specimen containers must be labeled with patient’s first and last name.
• Send specimen with completed requisition which states collection site and clinical information.
